Background:Antimicrobial resistance is a growing public health issue affecting the majority of global population. There are many causes of antimicrobial resistance (AMR), including drug-resistant bacteria and poor public health care. This study aims to evaluate the effectiveness of Zithromax and Azithromycin in the treatment of uncomplicated UTI and to evaluate the overall effectiveness and cost-effectiveness of the combination of these two medications in treating this condition.
Methods:The effectiveness and cost-effectiveness of Azithromycin and Zithromax were evaluated using a retrospective cohort study (N = 818,anus, chlamydia, and gonorrhea diagnoses). The study was conducted between 2009 and 2020 in two private clinics in the United States. Patients were identified as infected with either a single or multiple resistant (MDR) bacteria. Outcomes of treatment were measured using the antibiotic resistance index (ARI), which is a tool that compares the resistance rates to the resistance rates in a patient's general population. In addition, patients were randomly assigned to treatment with Zithromax or Azithromycin. The primary outcomes of interest were ARI and the cost of treatment with Zithromax. The costs of treatment with Azithromycin and Zithromax were $20,000 per patient, compared with $20,000 for the single agent treatment of gonorrhea, $40,000 per treatment, and $60,000 per patient, for the combined treatment. Outcomes were also measured using the cost of treatment with Zithromax and the cost of treatment with Azithromycin.
Results:The cost of treatment with Zithromax and Azithromycin was $28,000 per treatment and was significantly higher for patients with chlamydia, gonorrhea, and rectal UTIs compared to those with uncomplicated UTI or chlamydial infections (p <0.001). The total cost of treatment with Zithromax was $42,000, compared with $30,000 for azithromycin, which was a significant cost saving (p <0.001). The cost of treatment with Azithromycin and Zithromax was more effective in treating uncomplicated UTI, especially in those with a history of chlamydia, gonorrhea, and rectal infections. The cost-effectiveness of Azithromycin and Zithromax, compared to azithromycin, was not statistically different in either treatment group.
Conclusions:Overall cost-effectiveness analysis demonstrated that both Zithromax and Azithromycin have a high overall effectiveness and cost-effectiveness in treating uncomplicated UTI and chlamydia infections. The combination of these two medications has a high overall cost and should be considered when treating these infections in patients with a history of UTI.
